Mental Health Modesto LLC uses health information about you for treatment, to obtain payment for treatment, for administrative purposes, and to evaluate the quality of care that you receive. Your health information is contained in a medical record that is the physical property of Mental Health Modesto LLC.
We also describe your rights and certain obligations we have regarding the use and disclosure of health information.
Mental Health Modesto LLC may use your health information to provide you with medical treatment and services. For example, a health care provider — such as a physician, nurse, or other person providing health care services to you — will record information in your record that is related to your treatment. This information is necessary for health care providers to determine what treatment you should receive. Health care providers will also record actions taken in the course of your treatment and note how you respond to those actions.
Mental Health Modesto LLC may use and disclose your health information to others for the purpose of receiving payment for treatment and services that you receive. Mobile information and SMS consent will not be shared, sold, rented, or disclosed to third parties or affiliates for marketing or promotional purposes. We may share mobile information with service providers only as necessary to deliver SMS messages on our behalf, support our communications systems, or comply with applicable legal obligations. These service providers are not permitted to use your mobile information for their own marketing or promotional purposes.
You may opt out of receiving SMS messages at any time by replying STOP to any text message you receive from us. After you opt out, you may receive one final confirmation message, and we will no longer send SMS messages to that mobile number unless you opt in again.

Mental Health Modesto LLC may release relevant health information to a family member, friend, or anyone else you designate in order for that person to be involved in your care or in payment related to your care. Mental Health Modesto LLC may also disclose health information to those assisting in disaster relief efforts so that others can be notified about your condition, status, and location.
Mental Health Modesto LLC does not use your information for fundraising unless authorized, in writing, by you.
